Dual charging station VMA4400C lights turn off within 10 seconds

My dual charging dock is crazy.

Symptoms:
Lights turn yellow upon inserting battery. Within 10 secs turns off. No green light or anything.
I got that far by using a suggestion from another forum post; I unplug the charger and upon plugging in the charger I inserted the batteries within two minutes.

Batteries charge fine in the devices.

Is this happening when plugging in to a different power outlet?

It happens using any outlet. I also tested both of the cords and AC adapters that came with my kit from Costco.
