Are you going into "Add device", and then selecting the Pro 4 camera?
No need to scan a QR code on the iPhone. If you are connecting the camera to your home wifi, the install process will put a QR code on the phone screen that the camera needs to scan.

I have a hand held hot spot box it’s not for my home it’s for my barn ina different town I want to connect it to my iPhone so I can see what is going on in my barn and when I scan the QR code after I got to devices and click on arlo pro 4 it says searching for devises and then it comes up after about 2 min not device found

Make sure it's supplying a 2.4GHz. SSID and that the password doesn't have special characters, including spaces. What encryption is the hotspot using for the WiFi? Is your phone also connected to the hotspot?

It’s on 2.4 and yes my phone is connected to the same hot spot it was connected twice before and it kept going off so I switched it to home so it wouldn’t keep going off during the day cause of everyone walking by the camera and then it said my camera was offline

It's possible the hotspot is turning off to save bandwidth. You might want to check with your cell provider as well as the hotspot's manufacturer to see what they say.
