My wife and I have two Pro 4 systems at two different homes, both with the VMB4540 base. We were both able to set up the app on our iPhones for the original system at home #1. When we added an identical system to home #2, I was able to do the setup normally and add the #2 base to my app without difficulty, however, my wife cannot. Her phone will not detect the #2 base and we've tried all of the troubleshooting recommended via the app. Any suggestions? Do I need to do something on my end to allow her access?

Yes.  You need to share the cameras on the second base with her account.

 

This is assuming you are using the primary account and your wife is using a "friend" account.
