I am trying to get access thru the web.  When it sends a security code notification to my phone I never get it.  I looked in various places including 'notifications'   If it really has been sent where will I find it?  or does it go to my email?

The default is "web push", which should show up on the phone if the app is running.

After the verification times out, you can "select another verification method" and choose email.  Once in you can add SMS (Text) verification if you like.
