End of Life Order Form - Link Not Working

I have a couple of products that are impacted by the EOL policy. Oddly they have been working great for years with battery life usually lasting weeks or months. Now all of a sudden I'm replacing batteries weekly, which is nuts.

I noticed the EOL Policy has a link to an order form that is the following URL:

https://www.arlo.com/en-us/eol-offer-form.html?arlo=eol

This link just redirects to the main page. Does anyone know if the EOL offers are still available?
